From a Midwife to a Changemaker: My Journey to Transforming Maternal and Reproductive Health care in Ghana.
I never imagined that my career as a midwife would lead me to start a social enterprise, but life has a way of directing us to where we are most needed. My name is Sarah Nkansah Boateng, and I am the founder of Talk to Your Midwife (TTYM) Global. My journey began in the labor ward, where I witnessed firsthand the struggles of pregnant women, particularly market women, who spend long hours in their stalls, unable to attend antenatal care. Their resilience was admirable, but their vulnerability to high-risk pregnancies was alarming.
In 2020, I was posted to the Eastern Region of Ghana to work. I was confronted with a devastating truth, maternal mortality in the region stands at a striking 103 per 100,000 live births which means 9 mothers die every day due to complications in pregnancy and childbirth. These are not just statistics; they represent real women; mothers, daughters, and wives whose lives could have been saved with timely antenatal care and proper health education.
At the same time, I saw another pressing issue; adolescent girls lacking access to sexual and reproductive health (SRH) education and services. Without proper knowledge, many of these girls become victims of teenage pregnancy, some even losing their lives due to complications. The cycle of ignorance and vulnerability was heartbreaking. I knew I had to act.
TTYM started as a simple idea—a radio program aimed at educating pregnant women and adolescent girls about reproductive health. This initiative begun in 2021. Every week, I spoke on air, breaking down complex medical topics into relatable discussions, answering questions, and giving women the knowledge they needed to make informed decisions. The impact was immediate. I reached over 50,000 women and girls, empowering them with the right information to take charge of their reproductive health.
However, over time, I realized that education alone was not enough. These women needed access to healthcare, not just information. That’s when TTYM evolved into something bigger, revolutionizing maternal health and sexual and reproductive health by leveraging technology and community-driven initiatives. We built on existing maternal health models integrating innovative approaches which makes care more accessible and comprehensive.
Our social enterprise was launched in 2023 and today, TTYM Global is making measurable impact: We currently operate at the Agartha Market in Koforidua, Eastern Region, bringing antenatal care directly to the women who need it most providing immediate antenatal check-ups, risk assessments, and referrals, ensuring pregnant women can access care without leaving their businesses.

We have served 110 mothers since launching our Point-of-Care Service, ensuring they receive essential maternal health services like scan testing and laboratory investigations. Through early screenings, we identified five high-risk pregnancy cases and successfully referred them to our partner clinics for specialized care. Our collaboration with the Eastern Regional Ghana Health Service has strengthened maternal healthcare efforts in the region. According to the Eastern Regional Ghana Health Service’s 2024 annual report, our initiative contributed to a 3% increase in antenatal attendance; a great step in the right direction.
My work is more than a career; it is my life’s calling. I am driven by the stories of the women and girls I meet every day. The mother who finally has a safe delivery because she attended our point-of-care service. The teenager who avoids an early pregnancy because she received the right education. The countless women who now know they have a right to quality reproductive healthcare.
My passion is fueled by the belief that no woman should die giving life. Every mother deserves a chance to see her baby grow, and every girl deserves the opportunity to plan her future.
This journey is not without challenges—funding, cultural barriers, and systemic gaps often slow us down. But the impact we’ve made keeps us going. As we continue to expand, my dream is to scale and replicate TTYM Global’s point of care service in other market places in other regions of Ghana and even beyond, reaching women across Africa and beyond. I envision a world where maternal and reproductive healthcare is accessible to every woman, regardless of her social or economic background.
The work has just begun, but I remain committed. Because midwives don’t just deliver babies—we deliver change.
